Telecommunication, transportation, water, and wastewater companies are required to annually report information on operations and financials. The reports are due by April 1st of each year. Forms must be completed even if the company is reporting “zero.”
Separate notification is required via an Authorized Utility Representative Form for changes in company contact information -- i.e., name, address, telephone number, contact names, sale or purchase of company, corporate structure.
Submitting the Annual Report
One copy of the Annual Report should be submitted to the PSC email address. In addition, one copy should be retained by the company. Filing an electronic copy with the PSC will satisfy the utility's responsibility for submitting an Annual Report as required pursuant to PSC regulations.
Failure to comply with the submission of the Annual Report may result in fines and/or revocation of Certificate of Public Convenience and Necessity.
